Community Health

The Community Health team supports health improvement and substance use prevention strategies at the local and county level. We work in collaboration with the members of the Choose To Be Healthy Coalition and the Youth Resiliency Coalition, which are funded through state and federal grants.

Coalition partners include individuals, municipalities, schools, civic organizations, businesses, the faith community, and law enforcement. While most of the work is done within the York Hospital service area, support is also provided to York District Public Health (York County).

2021–2024 Community Health Needs Assessment

York Hospital conducted its most recent Community Health Needs Assessment (CHNA) for Southern York County in 2021 to inform community health efforts for the coming three years. We identified mental health, substance abuse, and access to care as three issues to prioritize.
